SAGE Specialist (half-time)
The following half-time position is available for Hudson Pride Connections Center’s supportive and outreach programming addressing the social service and advocacy needs of LGBT seniors as the northern NJ affiliate chapter of the national SAGE organization.
Hours:
20-25 hours per week average
Responsibilities:
- Responsible for the on-going development and implementation of SAGE supportive programs and activities including, but not limited to support group(s), care management, senior/community educational events and senior social activities such as bi-monthly dances, day trips and outdoor trips;
- Responsible for task supervision and support of SAGE volunteers assisting in the on-going delivery of SAGE programming, including services, events and social activities
- Responsible for:
- assisting ED with the development of HPCC-SAGE program marketing materials, including preparation of monthly SAGE-NNJ newsletters and routine/timely posting of program descriptions and calendar of events onto website;
- marketing HPCC-SAGE services, events and activities to community and providers via in-person, web-based, e-mail-based and postal mail-based outreach methods;
- monitoring SAGE Northern NJ website page to ensure timely response to public requests for information and/or assistance related to LGBT seniors;
- processing client intakes;
- conducting client needs assessments to inform planning of care management strategies;
- organizing client focus groups and community needs assessments to plan, schedule and deliver new programs, events and social activities and inform future program expansions;
- locating available community resources for seniors in general;
- assessing and rating senior programs and senior-serving agencies for LGBT-sensitivity;
- making client (and client family member) referrals to LGBT-sensitive programs/agencies;
- advocating for LGBT-sensitivity for unique senior programs with low ratings;
- conducting educational outreach to area providers about HPCC-SAGE and LGBT senior needs;
- co-facilitating senior wellness support group;
- organizing senior social activities
- Responsible for documenting, tracking, and monitoring client program utilization records of daily activities related to delivering services, holding events, providing education and/or reaching out to clients, community and/or providers in order to create program reports for grants, funders, and organizational reporting;
- Responsible for maintaining on-going contact with clients, including follow-up for new clients or those who have not received services for more than 6 weeks;
- Responsible for client file maintenance;
- Responsible for meeting all SAGE program outcomes, including client satisfaction ratings;
- Responsible for facilitating and documenting monthly SAGE Northern NJ committee meetings;
- Responsible for representing SAGE-NNJ and HPCC on committees and at meetings/conferences;
- Work with Clinical Director on development and implementation of HPCC mental health counseling program, including placement of interns within SAGE programs and referral of clients;
- Provide direct support to the Executive Director;
- Perform other related duties as assigned by Executive Director
Qualifications:
- Undergraduate degree in Social Work, Psychology, Education or related field or alternatively, current college enrollment and extensive experience working with LGBT individuals;
- willingness to work flexible hours
- Knowledge of Hudson County/Northern New Jersey region;
- Computer literacy in word processing, databases, spreadsheets, Internet savvy;
- Quick learner with initiative;
- Ability to work independently and collaboratively;
- Experience with the population being targeted,
- Written and spoken fluency in Spanish desired but not necessary.
